We have provided here 3 options for you to submit your adoption request.

• An on-line form you can use to submit your request for adoption.
• An Adobe PDF file you can download, fill out by hand and send via general mail
• A Microsoft Word document
• Download the file to your own computer (it’s safe - NO virsuses)
• Open the file with a Microsoft Word software program.
• Note: The file was created with Microsoft Word 2000
• Attach the file to an email message and send to livineasy@centurytel.net.

Please note I spay and neuter all of my little orphan pets and ensure they are seen by my local vet to be current with all necessary immunizations and are ready for adoption. This cost is NOT passed on to adoptive guardians. We do accept donations to help with our local effort if you would like to be part of Perry’s Little Orphans activities.
